RN Clinical Manager - Home Health
Location: Fresno, CA
Job Type: Full-Time
Pay: From $60/hour
Schedule: 40 hours per week
Work Location: In Person (Field + Office-Based Home Health Role)

Key ResponsibilitiesOversee all patient care services and clinical personnel, including assignments, coordination of care, and referrals in compliance with Medicare CoPs and state regulations
Ensure individualized care plans are developed, implemented, and updated based on patient needs and physician orders
Supervise and support field clinicians (RNs, LVNs/LPNs, therapists, aides)
Travel to patient homes as needed to support clinical oversight, staff supervision, and care quality monitoring within the home health service area
Conduct case conferences and promote interdisciplinary collaboration
Review clinical documentation for accuracy, timeliness, and compliance
Lead QAPI initiatives and support improved patient outcomes
Participate in hiring, onboarding, training, and performance management of clinical staff
Manage scheduling, caseloads, and visit coordination to ensure operational efficiency
Address patient, family, and referral concerns in a timely and professional manner
Collaborate with leadership on policies, procedures, and operational goals
Provide ongoing clinical leadership and support during operating hours
Active RN License preferred
2-3 years of recent clinical experience in home health or community-based care
1-2 years of leadership/supervisory experience in home health preferred
Strong knowledge of Medicare CoPs, OASIS, and home health documentation standards
Experience managing caseloads and interdisciplinary care coordination
Ability to coach and develop clinical staff
Familiarity with EMR/EHR systems and reporting tools
Strong communication, organizational, and time management skills
